This agreement releases Stars Cheerleading / Lively Arts from all liability relating to injuries that may occur during Bring a Friend night at Elite Stars Cheerleading. By signing this agreement, I hereby certify that I am the adult parent or guardian of the minor child listed above. I agree to hold Elite Stars Cheerleading / Lively Arts entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I understand and acknowledge that I am fully aware of and assume the risks of said minor child’s participation in cheerleading. Additionally, said minor does not have any conditions that will increase the likelihood of experiencing injuries while engaging in this activity. By signing below I forfeit all right to bring a suit against Elite Stars Cheerleading / Lively Arts for any reason.
